Brookestoneacrescare is seeking compassionate LPNs and RNs to support a collaborative care team focused on residents’ needs and high-quality care. You will lead and support the clinical team, ensuring safe, person-centered care for residents in a community setting.

Key responsibilities include medication administration, resident assessments, coordination with CNAs and other providers, supervising staff, and maintaining detailed care documentation to promote a positive experience for residents
