Job summary

Phoenixhomehc is seeking a Weekend Hospice RN or LPN for full-time on-call weekends in St. Louis, MO and surrounding areas. The role coordinates and provides patient care using the nursing process, ensuring adherence to professional standards and collaboration with the interdisciplinary team.

The position includes benefits, on-going training, and a supportive hospice culture. Missouri or compact license and valid driver’s license are required. Previous hospice experience is preferred.

Qualifications

• Experience in hospice care preferred.
• Missouri or Compact Nursing License required.
• Strong organizational and self-management skills.
• Valid driver’s license.

Responsibilities

• Skilled visits to patients in their homes or facilities.
• Day-to-day patient interaction and care coordination.
• Education on health conditions (CHF, COPD, diabetes).
• Wound care and dressing changes as prescribed.
• Medication management and safe administration.
• Document patient conditions and report changes to physician.
• Collaborate with nurses, facility staff, physicians, and hospice consultants.

The Weekend Hospice Nurse is responsible for coordinating and providing patient care using the nursing process, which includes intake, assessments, planning, implementation, and evaluation. Moreover, they must ensure adherence to professional nursing standards and maintain clinical competency while effectively collaborating with patients, their significant others, and members of the interdisciplinary team.

Responsibilities:

• Skilled Visits
• Day-to-day patient interaction
• Education on Health Conditions, such as CHF, CODP, Diabetes
• Wound Care
• Medication Management
• Changes dressings, bandages and contraptions as prescribed
• Documentation of patient’s condition and pointing out significant changes, to be reported to the physician.
• Working alongside other nurses, facility staff, physicians, intake coordinators, and hospice consultants

Requirements:

• Previous hospice experience preferred.
• Missouri or Compact Nursing License
• Strong organizational and self-management skills
• Valid driver’s license
