We are seeking dedicated Registered Nurses (RN) or Licensed Practical Nurses (LPN) to provide compassionate, comprehensive patient care across day and night 12-hour shifts. Deliver direct patient care by assessing needs, developing care plans, and implementing interventions aligned with best practices and regulatory standards.
Administer medications, including injections and IV infusions, ensuring accuracy and adherence to protocols.
Monitor vital signs, perform physical examinations, and document findings accurately within electronic health record (EHR) systems.
Assist with diagnostic evaluations such as specimen collection, processing, and interpreting results for appropriate clinical action.
Manage complex cases involving acute pain, infectious diseases, dementia or Alzheimer's care, and trauma-related conditions.
Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and long-term care strategies for optimal patient outcomes.
Maintain strict compliance with HIPAA regulations and clinical confidentiality policies while ensuring a safe environment for patients and staff.
Provide education and support to patients and their families regarding medication administration, disease management, and health coaching.
Valid RN or LPN licensure with current state certification; additional certifications such as BLS (Basic Life Support) are required.
Proficiency with EMR/EHR systems like PCC; familiarity with ICD coding (ICD-9/10) is preferred.
Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to diverse patient populations.
Experience working with individuals with disabilities or developmental delays; geriatric or memory care experience is advantageous.
Ability to perform advanced nursing procedures including catheterization, tube feeding management, sterile techniques, and ventilator support if applicable.
Excellent communication skills coupled with a compassionate approach to patient-centered care in fast-paced environments such as urgent care or skilled nursing facilities.
