Job summary

Methodist Le Bonheur is hiring a Registered Nurse to serve as a consistent care coordinator across the continuum of care. You will assess patients’ physical, psychological, and social needs and improve their care experience.

You will coordinate communication with providers and support education, screening, prevention, and follow-up care. The role requires nursing licensure in Tennessee or eligibility, an Associates degree in nursing, and preferably a Bachelors in Nursing.

Qualifications

• Associates Degree in Nursing is required.
• BLS/CPR certifications are required.
• Tennessee RN license or eligible licensure is required.

• Assesses the physical, psychological, and social needs of the patient, and improves the care experience for the patient and their family.
• Creates a seamless flow through the health system to facilitate the patient’s care.
• Develops and coordinates education, resources, and activities that provide understanding and support for patients and families.
• Maintains reliable systems to document, track and monitor patient data.
• Performs clinical procedures within the scope of nursing practice and clinical training.
• May perform telephone duties within the scope of RN role.
• Performs other job functions as assigned or requested.

Education/Formal Training Requirements

• Required - Associates Degree Nursing
• Preferred - Bachelor's Degree Nursing

Work Experience Requirements

• Required - Registered Nurse 0-1 years
• Preferred - Clinical nursing 1-3 years

Licenses and Certifications Requirements

• Required - BASIC LIFE SUPPORT - American Heart Association
• Required - Registered Nurse Tennessee - Tennessee Board of Nursing
• Preferred - Preferred: Certification in clinical specialty

Knowledge, Skills and Abilities

• Knowledgeable of system’s health resources in order to navigate the patient to appropriate care.
• Must have excellent verbal and written communication skills, a desire to problem solve, and strong conflict resolution abilities.
• Strong computer skills. Must be a role model in customer service and patient care.
• Displays sensitivity and compassion.

Physical Demands

The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion. Must have good balance and coordination. The physical requirements of this position are: medium work - exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently. The Associate is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading. Frequent invasive and non-invasive patient contact. Exposure to patient body fluids as well as exposure to hazardous or poisonous materials. Ability to react quickly to emergency situations. Ability to handle and administer medications, vaccinations and immunizations as needed.
