Job summary

Heart to Heart Hospice is seeking an on-call Registered Nurse to provide home visits for patient assessment, treatment, and support. You may be involved in crisis management, admissions, and ongoing care planning with the patient’s physician and IDG.

The role emphasizes strong communication with patients, families, and healthcare teams, timely documentation, and adherence to local regulations and company policies. A current RN license and valid driver’s license are required.

Qualifications

• Must be a graduate of an accredited nursing program (Diploma, ADN or BSN).
• Active State RN license and valid driver's license with reliable transportation.
• Two years RN experience preferred, healthcare/clinical setting preferred.

Responsibilities

• Provide home visits for patient assessment, treatment, and support, including crisis management and admissions.
• Develop and revise nursing care plans with patient physician and IDG.
• Maintain excellent customer service with patients, families, and staff using proper phone etiquette.

What You Must Have:

What You Will Do:

• Provide home visits as needed for patient assessment, treatment, and support. Visits may include pronouncement of patient death, crisis management and admission
• Plans for comprehensive service to patients and their families; develops and/or revise nursing care plans, including goals in conjunction with the patient’s physician and IDG
• Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
• Prepare clinical and progress notes that demonstrate progress toward established goals
• Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
