Job summary

Hartford HealthCare Medical Group is seeking an RN to provide skilled nursing care in diverse clinical settings under provider direction. You will assess, plan, intervene and evaluate patient responses using established protocols, and report to a Practice Manager.

Responsibilities include office-based nursing, triaging calls, coordinating care with providers, and supporting families with empathy while maintaining confidentiality. Cardiology experience is highly desirable.

Qualifications

• Active Connecticut RN license and CPR certification required.
• Experience in a medical practice setting preferred.
• Familiarity with medical software systems.

Responsibilities

• Provide office-based nursing care in collaboration with the provider.
• Assess patient needs, plan and evaluate care, ensure continuity of care.
• Demonstrate independent nursing actions based on assessment.
• Triages telephone calls and initiates appropriate nursing interventions.
• Assist patients and families with empathy while maintaining confidentiality.
