Qualifications

• Current CT RN license and CPR certification.
• Three years of experience in a medical practice preferred.
• Experience with medical software.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and formulates a care plan.
• Triages telephone calls and initiates appropriate nursing interventions.
• Assists patients and families with concern and empathy while maintaining confidentiality.
• Travel to other Hartford HealthCare regional offices, as needed.
