Job summary

Qualifications

• Organizational and multi-tasking skills required.

• Ability to use abstract reasoning to assess patients and adjust care plans.

Responsibilities

• Conduct comprehensive assessments of patients across physiological, psychological, developmental and sociocultural factors.
• Develop and implement individualized care plans and revise as needed.
• Administer medications and treatments safely per orders and protocols.
• Educate patients and families on conditions, treatments and self-care.
• Collaborate with medical staff to coordinate patient care and ensure effective communication.
• Document patient information and outcomes accurately in the medical record.
• Evaluate care effectiveness and adjust plans to improve health outcomes.
• Ensure infection control, safety standards, and regulatory compliance.
