Job Description
Purpose of Position : Delivers patient care through the nursing process of assessment, planning, implementation and evaluation.
Description
- Properly utilizes the nursing process in caring for patients by maintaining relevant, accurate and current patient records.
- Provides documentation of assessment of the patient.Summarizes assessment data and initiate plan of care with communication of pertinent data to allied patient care providers.
- Acts as resource person regarding telephone and in-person triage of patient medical needs for prioritizing treatment purposes.
- Participates in at least four staff meetings per year, attends mandatory programs.
- Review all test results received in OBGYN Department, review with provider for assessment and follow up instructions, document appropriately in patients’ medical record. Notify patient of results either by mail or phone, as needed/requested by provider.
- Review prescription refill requests with provider and coordinate refills, including initiating and completing prior authorizations as needed, as directed by provider.
- Provides patient care to post op patients including but not limited to staple removal, routine wound check, BP monitoring, Diabetes/insulin instruction/teaching, and administers injections and monitors patient responses to all medications given, as requested by providers.
- Provides patient care to established OB patients in the event that the providers are called to the hospital. (including but not limited to listening to fetal heart beat, measuring fundal height, discuss any patient concerns).
- Interviews newly pregnant patients for initial OB visit, including but not limited to documenting complete medical history and review of standard OB tests (lab and ultra sound) as well as available genetic testing. Provide patients with appropriate literature.
- Triage phone calls from patients, outside facilities, pharmacies, and other CCHC departments such as wet reads from Radiology. Acts as liaison between patients and providers, fielding questions and concerns and follow up, as directed by provider.
- Monitors quality measures; GYN test results including but not limited to biopsies, specimen testing, mammograms and ensure follow up is completed as directed by providers.
- Works with other offices within the community regarding mutual patient care and monitoring, including preparing paperwork as requested and directed by providers.
- Directs and guides patient care, teaching and activities of ancillary personnel while maintaining standards of professional nursing within the realm of Hospital policy.
- Works with Surgical Booking Clerk and providers, to complete any clinical aspects needed to obtain prior authorizations for procedures.
- Represents hospital in the community and within the hospital by maintaining confidentiality regarding patients and hospital matters and by presenting the hospital in a manner that will best market the services provided. 16.
- Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers.
- Performs other work-related duties and activities as assigned or requested by his/her immediate supervisor.
Qualifications
- Graduate of an accredited School of Nursing
- Currently licensed as a Registered Nurse in the Commonwealth of Massachusetts
- Basic Life Support Certification required
- Successful passage of CCHC’s Pharmacology exam
- Must read, write and communicate in English
- BSN Preferred
- Previous: clinical nursing experience with a minimum of 1-year experience in an OB and/or GYN setting
- Demonstrates proficiency in Electronic Medical Records for both hospital and outpatient office locations
Schedule Details : Per Diem, Monday-Friday, 8a-4p, 8:15a-4:15p, 8:30a-4:30p, No weekends and No holidays.
