Job Summary

A Registered Nurse (RN) in long‑term care is responsible for providing comprehensive nursing care to patients in a long‑term care setting. This role involves assessing patient needs, developing care plans, administering medications, and collaborating with healthcare teams to ensure optimal patient outcomes. The registered nurse will also educate patients and their families about health management and support them in daily living activities.

Primary Responsibilities

• Supervise and provide directions to Licensed Practical Nurse (LPN), Certified Med Aide (CMA), and Certified Nursing Assistants (CNA)
• Monitor and assess resident health needs and follow care plans
• Administer medications and manage treatments
• Provide wound care and follow infection control practices
• Collaborate with healthcare teams to ensure optimal outcomes
• Document nursing care, observations, and resident responses accurately and timely in EHR
• Maintain compliance with all state and federal regulations, including those related to infection control, resident rights, and safety standards
• Promote positivity, a compassionate environment that supports resident dignity and independence

• Proficiently read, write, speak, and understand the English language
• Strong problem‑solving and critical thinking abilities
• Strong verbal and written communication skills to interact with both patients and healthcare professionals
• Experience in patient assessment and care planning
• Proficiency in medication administration and management
• In‑depth knowledge of body systems and functions for effective patient health assessment

Qualifications

• Graduate of an accredited nursing program leading to licensure as a Registered Nurse (RN)
• Valid MN RN license
• Valid BLS certification
• Experience in a Long‑Term Care setting
• Must pass MN background check and fingerprinting
• Complete TB testing within the first three weeks of employment

Job Schedule/Location(s)

• Hallock, MN
• Shift options: 6:00am – 2:30pm, 2:00pm – 10:30pm, or 10:00pm – 6:00am
• Every other weekend and holiday rotation required
