Job summary

Collage Home & Community Rehabilitation Partners is seeking an experienced Registered Nurse (RN) to serve as a Nurse Supervisor for our Texas LHHS operations in Southlake. The role starts PRN and will quickly transition to full-time as client census grows, with RN-led clinical oversight and supervision of RNs, LVNs, and CNAs.

The ideal candidate will hold an active Texas RN license, have 2+ years of home health or complex care experience, CPR certification, and a valid TX driver's license.

Qualifications

• Active RN license in Texas
• Minimum 2 years nursing experience in home health, rehab, or complex care
• CPR certification required
• Valid Texas driver’s license and reliable transportation

Responsibilities

• Serve as the supervising RN for nursing and aide services under the Texas LHHS license
• Provide clinical supervision of RNs, LVNs, and CNAs
• Ensure CNA services are appropriately assigned, delegated, and supervised by nursing
• Conduct or oversee initial and ongoing patient assessments
• Develop, review, and update physician-ordered plans of care
• Monitor patient progress and respond to changes in condition
• Ensure services comply with Texas Health & Safety Code 142 and 26 TAC 558
• Maintain accurate, timely clinical documentation
• Participate in audits, surveys, and quality improvement activities
• Support intake, operations, and leadership to onboard new clients
• Help establish scalable clinical workflows as census grows
• Serve as a clinical resource to staff and leadership
