WellSpan Health is seeking a Nurse Navigator to guide patients across the continuum of care within defined episodes. The role emphasizes education, care coordination, and clinical expertise to support treatment plans.

The nurse will collaborate with interprofessional teams, advocate for patients, and ensure safe, efficient transitions across services, including community resources and home care as needed.

Qualifications

• Bachelor's degree in nursing is required.
• Minimum 5 years of nursing experience is required.
• Licensed Registered Nurse upon hire is required.
• Registered Nurse with multi-state license upon hire is required.
• Certification in appropriate nursing specialty preferred.
• Proven outstanding interpersonal skills; demonstrated ability to function as a team player.

Responsibilities

• Collaborate with interprofessional teams to develop and manage patient care plans.
• Provide education on disease processes, treatment modalities, and care plans for patients and families.
• Act as liaison between patients, families, healthcare teams, community resources, and entities to ensure seamless care provision.
• Facilitate open communication within interprofessional teams through verbal and written channels.
• Assist patients and families in scheduling ancillary testing and other necessary services throughout the care continuum.
• Plan for continuing care, including community services, home care, and hospice services as required.
