Description:RN Nurse Navigator position available.This role requires working day shifts, 5 days a week.No weekend shifts are required.Experience in case management and navigation is a must.The candidate should be proficient in various nursing tasks such as authorization forms, chart preparation, and follow-up phone calls.Responsibilities also include insurance verification and medication refills.Pain assessment, patient health questionnaire/history/allergies, and patient teaching are essential aspects of the role.Pharmacy calls and patient refill requests will also be part of daily duties.Additional responsibilities include referrals and scheduling appointments.Candidates should have experience with the Epic charting system.The role involves working with adult patient populations.Interdisciplinary support may be part of the work environment.Knowledge of patient diagnoses such as sickle cell disease is beneficial.This position does not require floating to other units or call obligations.
