Job summary

Live Well Homecare Agency, LLC is seeking an experienced registered nurse to provide advanced nursing support within an interdisciplinary team. You will promote preventative care, conduct screenings, and coordinate disease management while guiding patients and families in plan development.

You will triage messages, administer medications, vaccines, and IVs under provider direction, and ensure accurate documentation in EHRs. Strong communication and collaboration are essential.

Qualifications

• Minimum of 2 years RN experience.
• Graduate from a specialty nursing program; Bachelor's preferred.
• Strong clinical, communication, educational, and collaborative skills are essential.

• Assess healthcare, educational, and psychosocial needs; involve patients and families in treatment planning.
• Facilitate communication among healthcare team members to address barriers and interventions.

• Participate in professional development, review current literature, and incorporate new strategies into practice.
• Support post-discharge care transitions, assist in program design, and expedite lab specimen collection.
