Job summary

Hackensack Meridian Health in New Jersey is hiring a Registered Nurse Non-Acute to deliver direct patient care as part of an interdisciplinary team and coordinate care across services.

The RN will support Heart Failure physicians, with potential travel up to two days to Edison, Clifton or Glenridge locations. Schedule is Monday through Friday, 8:30 am to 5:00 pm. BSN in progress required within three years of hire; NJ RN license and BLS certification expected.

Qualifications

• BSN in progress and/or willing to acquire within three years of hire.
• NJ State Professional Registered Nurse License and BLS certification as required.

Responsibilities

• Deliver nursing care according to evidence-based research, policies and standards to ensure safe, individualized patient care.
• Assess data on patients' health, perform age- and disease-specific assessments, and identify education needs.
• Develop and coordinate patient/family-centered plans of care.
• Communicate and coordinate plan of care with the care team and patient family; document care.
• Administer medications safely using current bar-scanning/documentation technologies.
• Support discharge planning and transitions of care, and identify opportunities to enhance patient satisfaction.
