Job summary

Bend Transitional Care is a 60-bed skilled nursing and rehabilitation facility seeking a dependable, compassionate Full-Time Registered Nurse (RN) to join our night-shift team. You’ll provide safe, individualized care and support residents on their recovery journey.

Applicants should be a graduate of an accredited School of Nursing with current CPR certification; six months of long-term care experience is preferred.

Responsibilities

• Provide high-quality nursing care with sound clinical judgment.
• Supervise and support nursing staff.
• Assess residents and monitor changes in condition.
• Develop and implement individualized care plans.
• Safely administer and manage medications and treatments.
• Communicate changes in resident condition with physicians, families, and care team.
• Complete accurate nursing documentation, assessments, orders, progress notes, and flowsheets.
• Monitor resident responses to medications and treatments and report adverse reactions.

What You’ll Do

As an RN at Bend Transitional Care, you’ll play a key role in providing safe, compassionate, and individualized care to our residents. You will:

• Provide high-quality nursing care and demonstrate sound clinical judgment
• Supervise and support nursing staff
• Assess residents and monitor changes in condition
• Develop and implement individualized care plans
• Safely administer and manage medications and treatments
• Communicate changes in resident condition with physicians, families, and members of the care team
• Complete accurate and timely nursing documentation, assessments, orders, progress notes, and flowsheets
• Monitor resident responses to medications and treatments and report adverse reactions
• Maintain accurate narcotic records and ensure medication orders are followed
• Respond to emergencies, initiate appropriate interventions, and perform CPR when needed
• Protect resident rights and maintain confidentiality
• Follow federal and state regulations, the Nurse Practice Act, and company policies and procedures
• Maintain regular and reliable attendance

Licensure & Certifications
