Job summary

Cypress Springs Wellness & Rehabilitation in Mount Vernon, Texas is seeking a compassionate Licensed Vocational Nurse (LVN) for the night shift to deliver high-quality patient care to residents.

The ideal candidate will have a Texas LVN license, a HS diploma (or equivalent), and at least 1 year of LVN experience in a long-term care or rehabilitation setting. Strong teamwork and documentation skills are essential.

Qualifications

• Current LVN licensure in the state of Texas.
• High school diploma or equivalent required; associate's degree or higher in nursing or a related field preferred.
• Minimum 1 year of LVN experience in a long-term care or rehabilitation setting (n/a).

Responsibilities

• Provide direct patient care to residents under supervision of a registered nurse, including administering medications, wound care, and treatments.
• Conduct initial and ongoing assessments of patients' physical and emotional needs.
• Develop and implement individualized care plans to meet the unique needs of each resident.
• Work collaboratively with the interdisciplinary team to ensure comprehensive care and support.
• Document patient care and progress in accordance with facility policies and regulations.
• Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction.
