Qualifications

• Graduation from a state-accredited school of professional nursing.
• Licensed to practice as a Registered Nurse in North Carolina.
• Five (5) years of professional nursing experience.

Responsibilities

• Assesses patient status of a specialized population and coordinates ongoing acute and long‑term care needs.
• Delivers in‑services and education to nursing, medical and staff on topics related to the patient population.
• Collects and analyzes data for trends; participates in performance improvement activities and care protocol updates.
• Coordinates discharge planning and post‑hospitalization care, including referrals and coordination of clinics and home care.
• Promotes ongoing professional development and adherence to nursing process and professional standards.
