Job summary

SSM Health in Missouri is seeking an experienced RN Navigator to coordinate care across the continuum in a clinic setting. You will connect patients with resources, collaborate with physicians and care teams, and help promote positive outcomes and patient satisfaction.

Day shift, Monday-Friday, with no nights or weekends. Requires RN licensure and BLS certification; three years of RN experience preferred. Strong communication and organizational skills are essential.

Qualifications

• Graduate of accredited nursing program or equivalent licensing.
• Three years' registered nurse experience.
• Active RN license in applicable states.

Responsibilities

• Serve as liaison among management, physicians, and care teams to coordinate patient care.
• Collaborate with physicians and advanced practice providers to ensure quality outcomes.
• Collect data, track outcomes, and suggest process improvements.
• Provide referrals for follow-up care for underinsured patients.
• Apply nursing process (assess, plan, intervene, evaluate) in patient encounters.
