Job summary

SSM Health is seeking a compassionate Registered Nurse with pediatric experience to fill a part‑time Danis Peds Nurse Navigator position. The role involves navigating patients through the continuum of care, coordinating with physicians and care teams to ensure quality outcomes.

The RN will support follow‑up care, assist in education and referrals, and participate in data collection to drive process improvements. Evening and day shifts are possible within standard business hours.

Qualifications

Danis Peds Nurse Navigator – Part‑time Position

Work type: Regular. Schedule: Two 8‑hour shifts and one 4‑hour shift per week, between 8AM and 5:30PM Monday through Friday. Alternate schedule option: during a two‑week pay period, first week 3 8‑hour shifts and second week 2 8‑hour shifts.

Job Summary

Provides services as a patient navigator throughout the continuum of care.

Primary Responsibilities
  • Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
  • Collaborates with physicians and advanced practice providers to ensure quality outcomes.
  • Coordinates efforts for treatment and promotes quality improvement and educational efforts.
  • Compiles data, tracks outcomes, and makes recommendations for process improvement.
  • Provides referrals and support for follow‑up care for patients who are uninsured or underinsured.
  • Utilizes the nursing process of assessment, planning, intervention, and evaluation in all patient encounters.
  • Assists medical provider with processing of diagnostic results, bringing significant values to the provider’s attention.
  • Exhibits knowledge of tests and procedures and their significance as related to patient care.
  • Recognizes the implementation of dosages, interactions, side effects, adverse effects, routes of administration of drugs as they apply to individual patients.
  • Uses knowledge of medications in instructing patients about medication therapy.
  • Establishes working relationships, teaching protocols for patient education, family counseling, and general public information.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.
Education
  • Graduate of an accredited school of nursing or educational equivalency for licensing.
Experience
  • Three years' registered nurse experience.
Physical Requirements
  • Constant use of speech to share information through oral communication.
  • Constant standing and walking.
  • Frequent lifting/carrying and pushing/pulling objects weighing 0‑25 lbs.
  • Frequent sitting, reaching, and keyboard use/data entry.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent use of smell to detect/recognize odors.
  • Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25‑50 lbs.
  • Occasional lifting/moving of patients.
  • Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.
  • Occasional driving.
  • Rare crawling and running.
Required Professional License and/or Certifications

State of Work Location: Missouri. Basic Life Support HealthCare Provider (BLS HCP) – American Heart Association (AHA). Registered Nurse (RN) – Missouri Division of Professional Registration or Registered Nurse (RN) issued by Compact State.

Work Shift: Day Shift (United States of America).
