Job summary

Community Medical Centers’ Neuroscience Institute seeks an RN Navigator to coordinate MS patient care, ensuring timely access to specialized neurological services and smooth transitions among providers.

You will work with physicians, APPs, rehabilitation and support teams to minimize no-shows, track progress, and support families through the treatment journey.

Qualifications include California RN license, BLS, and 3 years RN experience; BSN or national certification preferred.

Qualifications

• High School Diploma or equivalent (IEP Certificate) required.
• 3 years of RN experience.
• RN license to practice in California and BLS certification required; BSN or national specialty certification preferred.

Responsibilities

• Coordinate care for MS patients with physicians, APPs, rehab and support teams to ensure timely access to specialized neurological care.
• Facilitate care transitions, monitor follow-up needs and support patients with complex clinical and psychosocial challenges.
• Improve care coordination workflows and support quality outcomes for a specialized patient population.

Responsibilities

As the RN Navigator, you will serve as a central point of coordination for patients with MS, ensuring timely access to specialized neurological care and reducing barriers throughout the treatment journey. Working closely with physicians, advanced practice providers, rehabilitation services and support teams — you will facilitate care transitions, manage follow-up needs and support patients with complex clinical and psychosocial challenges. You will leverage tracking and navigation processes to promote continuity of care, monitor patient progression, and help ensure no patient is lost to follow-up. Additionally, you will contribute to the growth and success of the Neuroscience Institute by improving care coordination workflows, enhancing access to services and supporting quality outcomes for a highly specialized patient population.

Qualifications

Education & Experience

• High School Diploma, High School Equivalency (HSE), or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required

• 3 years of recent RN experience

Licenses and Certifications

• RN - Current and valid Registered Nurse license to work within the state of California required

• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required

• NATIONAL - National Certification related to specialty and applicable to role OR Bachelor of Science in Nursing (BSN) Degree required
