Responsibilities

• In conjunction with the Medical Plan of Care, independently performs systematic and continuous assessment of the physical, emotional, clinical and psychosocial needs of assigned patients and families; plans and implements appropriate interventions; and evaluates effectiveness and adjusts plan as indicated in a timely manner in the provision of safe and effective care.
