About RN Mobile MDS Coordinator, Registered Nurse

CommuniCare Health Services is currently looking to fill the key position of Mobile MDS Coordinator, RN for its facilities in the Cincinnati/Dayton/Columbus, OH region. The successful candidate must have a valid RN license in Ohio, with previous Long Term Care and MDS experience. Multi-facility MDS or Senior MDS experience is preferred. The Mobile MDS Coordinator should be detail-oriented, a great team worker, and able to communicate effectively with residents, families, and staff at all levels. Travel throughout the region is required.

Preferred Responsibilities

• Current RN license in Ohio.
• Prior Long Term Care experience required.
• At least 2 years of experience with Federal Medicare/Medicaid and Managed Care reimbursement system management, working with the MDS and RAI process.
• Senior MDS and/or multi-facility MDS experience preferred.
• AANAC certification highly preferred.
• Excellent communication skills.
• Ability to multi-task and oversee multiple facilities daily.

Responsibilities

• Evaluate facility compliance with federal regulations and utilization targets.
• Conduct weekly visits for intervention, audits, and training.
• Review resident records for coverage, utilization, errors, and assessments.
• Provide consultation on federal and state reimbursement systems.
