Job description

RN Mobile MDS Coordinator, Registered Nurse

CommuniCare Health Services is currently seeking to fill the key position of Mobile MDS Coordinator, RN for its facilities in the Cincinnati/Dayton/Columbus, OH region.

The successful candidate must have a valid RN license in Ohio and previous experience in Long Term Care and MDS. Multi-facility MDS experience or Senior MDS experience is preferred. The Mobile MDS Coordinator should be detail-oriented and a team player, capable of effective communication with residents, families, and colleagues at all organizational levels. This role requires travel across the covered regions.

POSITION REQUIREMENTS

  • Current RN license in Ohio
  • Experience in Long Term Care
  • At least 2 years of experience managing Federal Medicare/Medicaid and Managed Care reimbursement systems, including MDS and RAI processes
  • Senior MDS experience and/or multi-facility MDS experience preferred
  • AANAC certification highly preferred
  • Excellent written and verbal communication skills
  • Ability to multitask and oversee functions at multiple facilities daily
  • Must maintain current CPR certification

Responsibilities include:

  • Evaluating facility compliance with federal regulations and utilization targets
  • Conducting weekly visits to facilities for intervention, audits, and training
  • Reviewing resident clinical records for coverage, utilization, errors, assessments, and accurate transmission
  • Providing consultation and expertise on federal and state reimbursement systems
