Job summary

Stoney River Ramsey is seeking an RN Memory Care Coordinator to lead our Memory Care neighborhood, delivering person-centered care, developing individualized service plans, and partnering with the Director of Nursing and interdisciplinary teams to promote best practices in dementia care.

You will educate and mentor LPNs, CNAs, and care staff, coordinate family care conferences, ensure regulatory compliance, and help maintain a survey-ready, compassionate environment focused on independence and

Qualifications

• RN license (state of MN preferred) with dementia care experience
• Experience with residents with Alzheimer’s or other dementias strongly preferred
• Experience in Assisted Living, Memory Care, SNF, or similar setting

What You’ll Do (AKA: Your Memory Care Leadership Superpowers)

Lead the Memory Care Program:

• Serve as the clinical leader for the Memory Care neighborhood, ensuring high-quality, person-centered care.

• Develop, implement, and evaluate individualized service plans.

• Partner closely with the Director of Nursing, Executive Director, and interdisciplinary team to promote best practices in dementia care.

Champion Person-Centered Dementia Care:

• Support residents through all stages of dementia with dignity, patience, and compassion.

• Model and train staff on engagement, redirection, meaningful activities, and non-pharmacological approaches.

• Foster an environment that promotes independence, comfort, and emotional well-being.

Educate & Empower Your Team:

• Provide ongoing training, mentorship, and coaching to LPNs, CNAs, Caregivers, Med Passers, and universal workers.

• Ensure team members understand and follow dementia-specific best practices and regulatory requirements.

• Help build a positive, supportive, team-oriented culture.

Partner With Families:

• Build strong, trusting relationships with families through consistent communication, education, and support.

• Lead care conferences and guide families through care transitions.

Support Quality, Compliance & Safety:

• Maintain compliance with state regulations related to Memory Care programming and clinical documentation.

• Participate in audits, surveys, and quality improvement initiatives.

• Ensure the neighborhood remains survey-ready at all times.

What You Bring to the Table (Besides Your Dementia-Care Expertise)

Experience & Credentials:

• Current RN license in good standing (required)

• Experience working with residents with Alzheimer’s or other dementias (strongly preferred)

• Prior experience in Assisted Living, Memory Care, SNF, or similar setting

Skills & Strengths:

• Strong understanding of dementia progression and person-centered care approaches

• Compassionate leadership style with excellent communication skills

• Ability to train, coach, and inspire teams

• Highly organized with strong follow-through

• Proficiency in Microsoft Office and electronic medical records (PointClickCare experience preferred)

Physical Requirements:

• Ability to lift up to 30 lbs and assist with resident care as needed

• Comfortable with walking, bending, and standing throughout the shift

• Ability to maintain a flexible schedule, including occasional evenings/weekends based on resident or staff needs
