Qualifications

• Associate degree in Nursing required and BSN preferred.
• Experience in Medical/Surgical nursing in acute care is preferred.
• Proficient in assessment, planning, and patient education.

Responsibilities

• Performs comprehensive assessments and reassessments to determine nursing care needs.
• Develops and evaluates individualized plans of care.
• Administers medications and treatments per policy and orders.
• Documents assessments, interventions, outcomes, and education.
• Provides direct patient care including ADLs, nutrition, and mobility.
• Monitors changes and communicates to the care team.
• Collaborates for discharge planning and referrals.
• Delivers emotional and psychosocial support to patients and families.
• Maintains infection control and fall prevention practices.
• Supervises tasks to licensed and unlicensed staff.
