Develops nursing plans of care with the patient and their designated care partner(s) to effectively address the individualized needs and priorities of patients, ensuring they align with their cultural beliefs and values.

Job Description

Essential Functions:

1. Interacts with co-workers, patients, their family/designated care partner(s), and other staff consistent with the values of Jefferson.
2. Integrates Evidence-Based Practice, clinical expertise, patient and family preferences, and cultural beliefs for optimal healthcare delivery.
3. Partners with patients and their family/designated care partner(s) to identify learning needs and priorities, providing education that is useful, understandable, and affirming.
4. Minimizes risk of harm to patients and others by following established nursing practices, protocols, and individual performance standards.
5. Uses information technology to communicate, manage knowledge, mitigate errors, and support ethical decision-making.
6. Uses data to monitor care outcomes and employs improvement methods to enhance quality and safety of patient care.
7. Works effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making.
8. Recognizes and respects patients and their family/designated care partner(s) in providing safe, compassionate, and coordinated care based on their personal preferences, priorities, needs, and cultural beliefs.
9. Creates an environment of belonging and inclusion, demonstrating problem-solving and conflict resolution skills.
