Job summary

LiveWell Homecare Agency, LLC in Colorado is seeking a dedicated Registered Nurse to provide direct patient care in a medical/physician office setting, applying the nursing process in line with state standards and the organization’s mission.

You will develop and evaluate care plans, monitor patient conditions, educate families, and collaborate with a multidisciplinary team to coordinate transfers, discharges, and psychosocial support; on-site, full-time, with pay based on experience.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Must hold valid RN licensure in Colorado and BLS certification.
• At least 6 months of nursing experience.

Responsibilities

• Develop, implement, and evaluate patient care plans.
• Monitor and document patient conditions.
• Collaborate across disciplines for care coordination, transfers, and discharges.
• Educate patients and families based on individual needs.
• Delegate tasks to ancillary staff; may precept students and new hires.
