Job summary

LiveWell Homecare Agency, LLC in Peoria, IL is seeking a registered nurse to deliver comprehensive in-home patient care using the nursing process to assess, plan, implement, and evaluate care.

You will collaborate with patients, families, and healthcare teams to develop individualized care plans, perform assessments, administer interventions, educate patients, and ensure safety and quality in accordance with policies.

Qualifications

• Valid RN license and BLS certification.
• Nursing diploma or college-level nursing education.
• Technical proficiency with medical equipment and lab testing.
• Strong communication, teamwork, and patient education skills.
• Ability to adapt to changing patient conditions and utilize resources effectively.

Responsibilities

• Promoting evidence-based practices and developing individualized care plans in collaboration with patients, families, and healthcare teams.
• Performing assessments, implementing interventions, and documenting patient progress to achieve desired outcomes.
• Managing clinical equipment, troubleshooting issues, and providing patient education tailored to age and comprehension levels.
• Coordinating interdisciplinary care, prioritizing tasks, and advocating for patients when necessary.
• Ensuring patient safety by adhering to policies, safety goals, and quality standards, while participating in process improvements and performance initiatives.
• Communicating effectively within the team and with patients, providing shift reports, and addressing patient/family needs promptly.
