Job Summary:

In this role, you will develop, implement, and evaluate individualized plans of care in alignment with patient priorities and interdisciplinary clinical standards. You will collaborate closely with physicians, care teams, and support staff to ensure open communication and continuous, high-quality care delivery.

Key Responsibilities:

• Comprehensive Patient Assessment: Evaluate physical, psychosocial, and educational needs of assigned patients to establish clinical baselines and care targets.
• Care Planning & Implementation: Formulate, execute, and adapt individualized nursing care plans based on ongoing clinical observations and physician orders.
• Clinical Interventions: Deliver evidence-based bedside interventions, administer medications, and perform clinical treatments tailored to individual patient care goals.

Qualifications & Requirements:
