Job summary

The Laurels of Walden Park is seeking an experienced Care Management Nurse, MDS Nurse, to manage the RAI process and coordinate care plans for residents. This role requires completing MDS, CAA’s and care plans within regulatory timeframes, and collaborating with physicians, nursing, social services, therapy, dietary, and activity staff.

Ideal candidates will have RN or LPN licensure and previous MDS nursing experience, with strong attention to regulatory requirements and interdisciplinary

• Experience as an MDS Nurse or in care management.
• Strong coordination with physicians, nursing, social services, therapy, dietary and activity staff.

Responsibilities

• Manages the RAI process and conducts assessments and care plan coordination.
• Completes MDS, CAA’s and care plans within regulatory timeframes.
• Assesses residents via physical exam, interview and chart review.
• Collaborates with care givers, physicians, and other staff on resident care needs.
• Reviews hospital and outside information to inform assessments and planning.
• Coordinates and initiates significant changes in MDS.
• Supports PPS meetings and monitors Medicare days and RUGs when needed.
• Stays current with AANAC requirements.

Responsibilities:

The Care Management Nurse, MDS Nurse works the RAI process and conducts assessments and care plan coordination for those residents assigned. Some responsibilities of the MDS nurse include:

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS.
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
• Remains current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications:

• Registered Nurse or Licensed Practical Nurse
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.
• Experience as an MDS Nurse.
