Job description

SUMMARY: Under the direction and supervision of the Clinical Manager/Director, the RN Assessment / MDS Coordinator is responsible for conducting and coordinating the completion of the resident comprehensive assessment which includes the Minimum Data Set (MDS), and for tracking and transmitting all data including the RUG levels to the Centers of Medicare & Medicaid Services (CMS). The process includes the coordination of appropriate participating health professionals (interdisciplinary team) for the purpose of conducting initial and period comprehensive, accurate assessments of each resident to plan care that allows the resident to reach his/her highest practicable level of physical, mental, and psychosocial functioning. The RN Assessment/ MDS Coordinator participates in departmental Performance Improvement Program. The RN demonstrates knowledge of Enhanced Standard Precautions and Isolation techniques. This position will serve as the back-up to the unit’s Clinical Manager/Director. Collaborates with all levels of unit nursing personnel, including the Clinical Manager/Director, physicians, consultants, department heads, residents, resident’s family or legal representative, and other disciplines determined by resident needs.

REQUIREMENTS

  • Minimum Experience: 6 months experience preferred
  • Required License/Certifications: Current CA RN, BLS, and MAB/ABM required (90 days upon hire). Other Certification required in designated units (listed below):
    • ER- ACLS, PALS & CPI
  • Must have the ability to meet essential functions of job with or without accommodation.
  • Maintains compliance with all state and federal laws and regulations, as well as other regulatory programs and hospital policies and procedures.
  • Serious about obligations and actions.
  • Takes ownership of tasks and completes assignments.
  • Meets mandatory requirements (Compliance and Ethics training attendance, annual competency assessment, TB screening, annual reorientation, etc.)
  • Successfully completes Company's compliance competency.
