Job summary

The Blossoms in Van Buren, AR is seeking a compassionate MDS Coordinator (RN) to perform federally mandated assessments in our long-term care facility. As an RN, you will collect data, compile thorough assessments, determine functional capacity, and help shape care plans and reimbursement decisions.

You will coordinate OBRA/PPS processes, lead Care Plan conferences, and collaborate with interdisciplinary teams to ensure timely reviews, accurate documentation, and compliance with CMS guidelines.

Qualifications

• Active RN or LPN license in good standing.
• Experience coordinating MDS/OBRA assessments preferred.
• Knowledge of CMS RUG-IV guidelines and billing basics.

Responsibilities

• Complete OBRA, PPS and Managed Care MDS coding and documentation.
• Coordinate Care Plan conferences within the 21/90 day schedule.
• Maintain accurate EMR entries and monthly OBRA calendars.
• Collaborate with interdisciplinary teams to ensure timely care plans.
• Monitor coding accuracy and regulatory compliance.
