Job summary

Catholic Health is seeking a Part Time MDS Nurse Coordinator for Father Baker Manor Sub Acute in New York. You will coordinate comprehensive resident assessments, support clinical staff, and help maintain high-quality life for residents in a 0.4 FTE, days schedule.

Required are NY RN licensure, nursing degree, RAC-CT certification (within 12 months), and ongoing RAC-CT maintenance. Prior residential facility experience is preferred.

Qualifications

• RN licensure in New York is required.
• Graduate of an accredited school of nursing.
• RAC-CT certification within one year from hire date.
• Maintain RAC-CT (MDS) certification.
• One year of experience in a Residential Health Care Facility preferred.

Responsibilities

• Coordinate comprehensive assessment for each resident.
• Support clinical staff to provide high quality of life for residents.
• Participate in the overall care process and coordination.
• Ensure RAC-CT/MDS compliance and documentation.
