Job summary

Cassia Moose Lake Village in Moose Lake, MN is hiring an experienced RN to join as a Full-Time MDS Coordinator. The role leads MDS and care planning, assigns nursing tasks, and ensures accurate RAI documentation. You will collaborate with Case Management, Admissions, and Supply to deliver resident-centered care.

Required: Minnesota RN license and Resident Assessment Coordinator certification. Prior LTC experience and proficiency with MDS systems are preferred.

Qualifications

• RN license in Minnesota is required.
• Resident Assessment Coordinator certification is required.
• Experience with MDS is preferred.
• Proficient in computer software.

Responsibilities

• Oversee and complete MDS and care planning.
• Manage and assign tasks to nursing staff on the unit.
• Complete all documentation for RAI and MDS assessments and observations.
• Develop initial and temporary care plans.
• Assist Case Manager with Medicare scheduling, monthly billing, charting, and notices of non-coverage.
• Document and maintain paperwork, referral records, and reports.
• Work with Admissions staff, Case Manager, and Supply Coordinator.
• Display a courteous attitude and respect for residents, families, and staff.
• Perform additional tasks as needed.
