Job summary

Majestic Care of Manchin seeks an MDS Coordinator (RN) to lead MDS and care plan coordination for residents, ensuring timely documentation and regulatory compliance.

The role includes supervising the MDS process, coordinating PPS and CAAS, and collaborating with care teams to maintain quality resident care while upholding privacy and rights. RN license required; LTC experience preferred.

Qualifications

• Must hold current RN or LPN nursing license in the state of employment.
• Experience in Long Term Care preferred.
• Computer skills such as Data Entry and Word processing required; working knowledge of the MDS 3.0.

Responsibilities

• Conduct and coordinate the MDS and Care Plan as outlined by facility policies and procedures.
• Inform care team when a care plan is due and ensure completion of MDS portions.
• Develop or revise resident care plans quarterly or with significant changes in condition.
• Coordinate all PPS and state MDS assessments and ensure CAAS/ MDS completion.
• Ensure confidentiality and privacy of residents' rights during MDS processes.
