Job summary

Adams Farm Living Inc. in Jamestown, NC is seeking a qualified MDS Coordinator to lead the MDS/PPS process, coordinate care plans from MDS/RUG classifications, and ensure timely, accurate documentation in accordance with state regulations.

You will work with the Interdisciplinary Team to assess resident needs, monitor MDS and care plan notes, and support RAI compliance using EHR systems such as PointClickCare. Strong communication, confidentiality, and organizational skills are essential.

Qualifications

• Active NC RN license required.
• Prior MDS and Case Mix experience.
• Experience with Electronic Health Records; PointClickCare preferred.
• Ability to communicate effectively with residents, families and all levels of staff.
• Ability to organize and prioritize workload.
• Maintain confidentiality at all times.
• Knowledge of RAI requirements.

Responsibilities

• Regulates and maintains the MDS/PPS program and completion schedule.
• Develops care plans from MDS/RUG.
• Evaluates effectiveness of care plans and updates MDS/RAI process.
• Collaborates with Interdisciplinary Team to assess resident needs.
• Monitors MDS and care plan documentation in the medical record.
