Bradley Estates Nursing and Rehab in Milwaukee is seeking an experienced MDS Coordinator. The role requires an active RN license and strong collaboration with care teams to promote timely, quality patient recovery.

You will monitor patient conditions, coordinate care plans, supervise LPNs and CNAs, and ensure compliance with health and safety guidelines while educating families. This role offers a supportive work environment and opportunities for professional development.

Qualifications

• A minimum of 1-2 years’ experience.
• A team player with excellent communication and interpersonal skills.
• Outstanding organizational and multi-tasking skills.
• Valid nursing license in the state of Wisconsin.

Responsibilities

• Monitor patient’s condition and assess needs.
• Observe and interpret patient’s symptoms and communicate them to physicians.
• Collaborate with physicians and nurses to devise individualized care plans for patients.
• Perform routine procedures (blood pressure measurements, injections, etc.) and fill in patient charts.
• Adjust and administer patient medications according to physician orders.
• Inspect facilities and maintain hygiene and safety standards.
• Supervise and train LPNs and nursing assistants.
• Expand knowledge by attending educational workshops and conferences.
