The Grove of La Grange Park is seeking an RN MDS Coordinator to join our caring team. You will develop and monitor resident care plans, perform assessments, and assist with discharge planning to ensure optimal outcomes for residents in a post-acute, skilled nursing setting.

Qualifications

• RN license in good standing
• Graduate of an accredited nursing program
• CPR certification required
• Prior experience as MDS Coordinator recommended
• Nursing home RN experience 1 year preferred
• 1 year CNA experience preferred

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan to meet individualized needs and goals
• Perform resident assessments and assist in discharge planning
• Ensure residents receive high-quality, compassionate care in a safe environment
