Job summary

Hanover Health & Rehabilitation Center in Mechanicsville, VA is seeking a full-time MDS Coordinator with an RN license to oversee MDS 3.0 assessments and care plan development in our 120-bed skilled nursing facility.

Qualifications

• RN license required and background in skilled nursing/rehabilitation.
• Experience with MDS 3.0 process and care planning.
• Strong communication and customer service skills.

Responsibilities

• Complete MDS assessments and create comprehensive care plans.
• Review patient records and coordinate with staff, physician and family.
• Ensure compliance with federal/state regs and company policies.
• Interact with an interdisciplinary team and manage daily operations.
