Fox Chase Healthcare is seeking an experienced Registered Nurse to fill an MDS Coordinator role in a skilled nursing facility. The MDS nurse will assess and collect information to support resident health, care planning, and regulatory compliance.

Formación

• Current Maryland RN license (or compact)</s>
• BLS/CPR certification
• In-depth knowledge of MDS and related guidelines
• Willingness to work in a skilled nursing facility and manage MDS processes

Responsabilidades

• Monitor residents' health and well‑being from admission through discharge using the MDS process.
• Plan, document, and submit compiled information.
• Communicate with staff, residents, and families to ensure care plans reflect needs.
• Maintain complete healthcare records and reports including PCC per best practices.
• Investigate patient/family complaints and resolve issues.
