Job summary

Ohana Pacific Health in Hale Kupuna, Koloa, HI is seeking a Full-time Minimum Data Set (MDS) Coordinator / RN to ensure timely, accurate MDS assessments, care plans, and electronic transmission for residents.

The role requires coordinating the Interdisciplinary Team (IDT) in the RAI process, reviewing MDS/Care Plans, and ensuring sign-off for completeness. Knowledge of RAC-CT and state regulations is essential.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum of 1 year of professional nursing experience.
• Current Hawaii RN license required.
• Maintain RAC-CT certification through AANAC.

Responsibilities

• Coordinate Interdisciplinary Team (IDT) in the RAI process per regulations.
• Review, edit, and sign MDS, CARE plans, and conference notes for completeness.
• Maximize reimbursement and quality of care with accurate MDS documentation.
