Job summary

Ohana Pacific Health in Hawaii is seeking a dedicated Minimum Data Set (MDS) Coordinator / RN in Lihue. This full-time role focuses on timely completion of accurate MDS assessments, care plans, and electronic MDS transmission for residents.

You will lead the Interdisciplinary Team (IDT) in the RAI process and ensure documentation reflects acuity and care provided. The ideal candidate holds an RN license in Hawaii, at least 1 year of nursing experience, and RAC-CT certification through AANAC.

Qualifications

• 1 year of professional nursing experience required.
• Graduate of an accredited school of nursing.

Responsibilities

• Coordinate IDT in development of RAI process per federal/state regulations.
• Review and ensure completeness/accuracy of MDS, CARE plans, and notes.
• Maximize resident reimbursement through accurate MDS review and documentation.
