Job description

Touchstone Communities is seeking an RN MDS Coordinator in Harker Heights, TX. This position plays a vital role in overseeing the MDS assessment and reimbursement process, ensuring compliance, accuracy, and quality care in a skilled nursing facility.

Responsibilities:
  1. Lead the MDS process to ensure accurate and timely assessments for Medicare, Medicaid, and case management reimbursement.
  2. Collaborate with clinical and financial teams to optimize reimbursement and compliance.
  3. Stay current on state and federal regulations related to RAI and reimbursement.
  4. Work with interdisciplinary teams to develop individualized care plans.
  5. Maintain accuracy in documentation, assessments, and coding.
Qualifications:
  • Active Texas RN license (or valid Compact State RN license).
  • Understanding of Medicare, Medicaid, and reimbursement systems.
  • Experience in Skilled Nursing Facilities as an MDS Nurse is preferred.
  • Knowledge of state and federal regulations.
  • Excellent organizational and communication skills with a detail-oriented approach.
