Summary of Position

The RN-MDS Coordinator is responsible for completing the RAI Assessment, ensuring the certification, accuracy, and completeness of the Minimum Data Set (MDS). The role exists to ensure MDS and care planning objectives are met.

Primary Duties & Responsibilities

  1. Attend seminars or training sessions to increase knowledge and competence, fostering good interactions within the interdisciplinary team.
  2. Conduct comprehensive assessments of residents' needs and capabilities based on standardized data, describing residents' functional abilities and impairments. Sign and certify assessments, coordinate findings with the interdisciplinary team, and electronically submit MDS to the State as required.
  3. Coordinate with staff and the interdisciplinary team to gather and analyze information for individual care planning.
  4. Develop comprehensive care plans for each resident, including measurable objectives and timetables to address medical and psychosocial needs identified in assessments.
  5. Perform nursing procedures and administer medications as needed, following the physician's care plan.

License/Qualifications

  1. State of TN or Multi-State License as an LPN or RN.
  2. Preferably, three (3) years of experience working with MDS in a Skilled Nursing Facility (SNF).
