Chestnut Ridge Health & Rehabilitation in Kentucky is seeking a compassionate and skilled RN MDS Coordinator to join our team and help deliver exceptional resident care through accurate MDS assessments and coordinated care planning. You will collaborate with nursing leadership to optimize patient outcomes and ensure compliance.

The ideal candidate is KY-licensed as an RN, holds a bachelor’s degree in nursing (preferred), and has about one year of MDS experience.

Qualifications

• KY RN license required or in process.
• Bachelor's degree in nursing preferred.
• Approximately 1 year of MDS experience preferred.

Qualifications:

• Currently licensed in KY as an RN.
• Bachelor's degree in nursing preferred.
• One year of MDS experience preferred.
