Job summary

American Senior Communities in Valparaiso, IN is seeking an MDS Assistant who will perform RAI completion and support the interdisciplinary care planning process in a licensed long-term care setting.

Responsibilities include ensuring regulatory compliance, contributing to person-centered care, and collaborating with nursing and therapy teams to optimize resident outcomes. Prior MDS experience and nursing experience in geriatric settings are preferred, with a strong commitment to CARE values.

Qualifications

  • Graduate of an accredited school of nursing.
  • Two years of professional nursing experience in long-term care, acute care, restorative care or geriatric nursing setting preferred.
  • Two years of professional nursing experience in nursing settings.

Responsibilities

  • Complete the Resident Assessment Instrument (RAI) and contribute to the interdisciplinary care planning process.
  • Ensure compliance with state and federal regulatory requirements.
  • Collaborate with the care team to support resident-centered care.

The MDS Assistant is responsible for the completion of the Resident Assessment Instrument (RAI) and the interdisciplinary care planning process while ensuring compliance with state and federal regulatory requirements.

Skills Needed:
  • Clinical Judgement/Assessment: Attention to detail and strong clinical assessment skills. The ability to develop plans of care that are consistent with the resident’s needs.
  • Collaboration: Promote communication and interdisciplinary approaches to resident care.
  • Supportive Presence: Create a comforting and engaging atmosphere for our residents.
Requirements:
  • Graduate of an accredited school of nursing.
  • One to three years of experience in MDS completion preferred.
  • Two years of professional nursing experience in long-term care, acute care, restorative care or geriatric nursing setting.
