Job summary

The Paramount at Somers Rehabilitation and Nursing Center is seeking an experienced MDS Assessor RN to join our team in Somers, NY. You will manage the RAI process, ensure accurate MDS 3.0 assessments, and support PDPM reimbursement while maintaining regulatory compliance.

Qualifications

• Active RN license in New York.
• MDS experience in skilled nursing facility or long-term care.
• Strong knowledge of MDS 3.0, RAI, and PDPM.
• Experience with Medicare, Medicaid, and case-mix reimbursement.
• Strong assessment, documentation, and analytical skills.
• Ability to work independently and as part of a team.

Responsibilities

• Complete and submit MDS 3.0 assessments (admission, quarterly, annual, significant change).
• Ensure CMS guidelines, state regulations, and Medicare/Medicaid requirements.
• Accurately document and code clinical information to support PDPM reimbursement.
• Coordinate with the interdisciplinary team (IDT) to support care planning.
• Maintain and monitor assessment schedules and ARD deadlines.
• Perform chart reviews and audits for accuracy and completeness.
• Stay current with RAI Manual updates and regulatory changes.
• Collaborate with nursing, therapy, and administrative staff to ensure quality care delivery.
