Develops nursing care plans with patients and their designated care partners to effectively address their individualized needs and priorities, aligned with their cultural beliefs and values.

Job Description

Essential Functions:
  1. Interacts with co-workers, patients, their families or designated care partners, and other staff, consistent with Jefferson's values.
  2. Integrates evidence-based practice, clinical expertise, patient preferences, and cultural beliefs for optimal healthcare delivery.
  3. Partners with patients and families to identify learning needs and priorities, providing education that is useful, understandable, and affirming.
  4. Minimizes harm by following established nursing practices and protocols.
  5. Uses information technology to communicate, manage knowledge, mitigate errors, and support ethical decisions.
  6. Utilizes data to monitor care outcomes and employs improvement methods for ongoing quality and safety enhancement.
  7. Works effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making.
  8. Provides safe, compassionate, and coordinated care based on respect for patient preferences, needs, and cultural values.
  9. Creates an inclusive environment, demonstrating problem-solving and conflict resolution skills.
  10. Demonstrates a commitment to professional growth and acts as a primary preceptor for staff development.
  11. Participates in hospital and practice setting committees, including those focused on safety and quality improvement.
  12. Welcomes and incorporates feedback into practice improvements.
