Job summary

The Lutheran Home, part of Lutheran Life Communities, seeks an Registered Nurse Manager (RN) to lead the Transitional Care Unit on a full-time day shift with on-call weekend rotation. Salary ranges $95,000 to $113,000 per year.

As the primary clinical coordinator, you will oversee care plans, collaborate with physicians and the interdisciplinary team, and drive quality outcomes while ensuring regulatory compliance and resident-centered care.

Qualifications

• Current Illinois RN license required.
• 2+ years of nursing experience in skilled nursing, transitional care, rehabilitation, or long-term care.
• Prior leadership or nurse manager experience preferred.
• Strong clinical assessment and critical thinking skills.
• Experience with interdisciplinary teams and care transitions.
• Ability to manage multiple priorities in a fast-paced environment.
• Commitment to resident-centered care.

Responsibilities

• Serve as the primary clinical coordinator for residents transitioning into and out of the Long Term Care or Transitional Care Unit.
• Oversee and manage clinical care plans to support recovery and discharge outcomes.
• Collaborate with physicians, therapy, case management, social services, and interdisciplinary teams.
• Monitor resident progress and address changes to prevent rehospitalizations.
• Educate residents, families, and staff on care plans and recovery goals.
• Support admissions, assessments, and care coordination for short-term rehabilitation residents.
• Ensure compliance with Federal, State, and Local regulations and policies.
• Participate in weekend on-call rotation to support clinical operations.
